At time of registration, parents or legal guardians must present their NJ Driver’s License and their child’s original birth certificate. The parent or legal guardian must present three (3) proofs of residency at the time of registration. Acceptable documents for this purpose are: utility bills; bank statements; property tax bills; house deed; contract of sale; rental lease (including names of children residing at the address); in the absence of a lease, a notarized letter from the landlord which includes all of the information found in a rental lease. In the case of divorced or separated parents, a copy of those pages of a court document indicating parental custody, including any limitation on communications with the non-custodial parent.

To be eligible to attend kindergarten this September, a child must be five years of age on or before October 1, 2009. Parents or legal guardians must present their NJ Driver’s License and their child’s birth certificate and medical record indicating that the child has received four DT&P inoculations; the fourth shot must be given after the fourth birthday. Five or more doses must meet the requirement. Three doses of Polio vaccine, one dose on or after the fourth birthday or alternatively, a child with any four (4) doses of Polio vaccine spaced by a minimum of one month will also be in compliance with this requirement. Students are required to have received two (2) doses of MMR, after one year of age or laboratory evidence of immunity. The two doses of measles-containing vaccine must be separated by an interval of at least one month. Additionally, children are required to receive three (3) doses of a hepatitis B vaccine and one (1) dose of varicella vaccine on or after the first birthday, or proof of disease immunity.

All students entering kindergarten will be required to submit evidence of a physical examination (before entering school.) Physical examinations cannot be dated earlier than one (1) year prior to entry and must be submitted on the approved district form stating what, if any, modifications are required for full participation in the school program.
