Hi. Greatschools is a nationwide forum, and we don't know where you live. When you say your young adult is out of college, is that to say he/she graduated, or he/she dropped out? I know there are some supportive College Living Experince programs in various areas, www.cleinc.net/
In my community, there is a "Special Recreation Association" which offers supervised social opportunties for young adults, such as bowling and trips to movie theaters for people who don't drive. You might contact someone at www.thearc.org in your state, and see if they can point you to additional resources.
