What can i do to get my 8th grader to stop forgetting homework?

My 8th grade son simply forgets to do any homework. We have no idea what his homework is becuase he doesn't write it down in his planner. We have talked to his teachers and they try to make sure they check his planner, but for whatever reason he doesn't bring them the planner 1/2 the time. He actually took an easy test the other day. He failed it because he didn't bother to read the chapter nor did he go through the easy review sheet the teacher provided the entire class 2 days before the test. It must bother him, but he doesn't let us know about it. We have to check up on him constantly to see what is happening. What can i do about this this? Any suggestions would be very helpful. HELP!!!!!!

make a chart with all the names of his classes in one column and then the next column is a place to write the assignment for each class and then a column for the teacher's signature and then a place for teacher comments.
This way you can ask to see the assignment sheet and make sure that the teacher "approved" what he wrote. This way he doesn't leave anything out of the assignment. So if there is "no homework" then the teacher has signed it saying that that is correct.
This way, you will know if he is telling the truth, and he becomes more accountable. You might want to let the teachers know you will be asking him to do this, the last five minutes of each class. That way, they can gently remind him.
Of course none of it works unless you check the assignment sheet and make him accountable.

When my kid forgets his work at school, I forget to let him play (video) games, or watch his television shows, I also forget to let him go out with his friends. I then remind him after the first time of forgetting things will start getting taken away. Then after that I will go with him to each class sit with him and will see how much he forgets. My kid has only gotten things taken away he hasn't gone that far but those are the things I tell him. (I have a 9 yr old boy)Sometimes our kids get lazy on us we just have to give them a little push or shove(haha) also maybe he is going through his puberty stages Do you think this is possbile? Please answer me I have 3 boys? :/ This could also be a way to get some attention? I don't know I am just throwing out some thougts. Could it be that maybe someone is bothering him at school, have you asked him that?

I hope some of this helps. Contact the school counselor, nurse, or vice principal. Tell them you are requesting an evaluation by the child study team . Report the difficulties you have had with memory and organization. Request that his teachers observe him and provide feedback for focusing and organization.
Make this request in writing. The school has to test and report to you (depending on your district) within 45 to 90 days. you want both educational and psychological testing. You may chose to go privately - but that is costly. The school has to do this at no cost to you. Also, talk to the pediatrician about these focusing issues. He might recommend medication.
