Here are some examples of ways to graph your child's standarized test scores (IOWA, state, woodcock johnson, etc.)  to create a visual demonstration  to the IEP team, school, district, etc., of your child's progress or lack of progress over their academic career.

GLE = Grade level equivalant

% or Percent = Percentile rank (typically the 50th percentile is dead average)

You can request a copy of all of your child's educational records both at the school they currently attend and the district (I typically do this incase one of my child's files either the school or district contains files that aren't in the other.

You can try verbally requesting them initially, if they don't provide you access/copies to your child's records either by ignoring your request or denying it, then you should send a certified letter return receipt snail mail addressed to the school principal.

IOWA, state test, should all be found in your child's ed file. The majority of the time if the child is in Spec ed their IEP and spec ed records including eval reports with scores etc. should be in the child's ed file, but if it's not, you may need to contact both your school and district's spec ed office.
